The University of Arkansas - Fort Smith Lions ID Card identifies the cardholder as a student, employee, or guest of the University, and upon receiving the card the cardholder agrees to the following terms and conditions:
1. AUTHORIZATION FOR USE
The Lions ID Card will not be loaned or otherwise transferred to another person, doing so results in a violation of University policies. Any attempt to obtain or use, or to assist in obtaining or using, a Lions ID Card for fraudulent identification or payment is a violation of the Student Conduct Code and/or state law. Any individual using the card fraudulently may be subject to disciplinary action.
2. PROPERTY OF THE UNIVERSITY OF ARKANSAS - FORT SMITH
The Lions ID Card is the property of The University of Arkansas - Fort Smith (UAFS). Upon a request from any UAFS official or security personnel, the cardholder will relinquish his or her Lions ID Card. In the instance of a request for refund of voluntary funds ONLY, upon withdrawal or graduation from the university; the cardholder agrees to return his or her Lions ID Card to the UAFS Registrar’s Office where they will be given a Request for Refund Form.
3. CARD EXPIRATION
If at any time a student or employee leaves UAFS, all privileges pertaining to the card will be removed and the Lions ID Card will be deactivated. These privileges will be restored and Lions ID Card reactivated upon that persons return to UA Fort Smith as a student or employee. If the individual returns to the University, and has not retained their Lions ID Card or changes to the physical appearance of the Lions ID Card require the issue of a new card, they will be charged the current Lions ID Card replacement fee.
4. CARD ISSUANCE
Lions ID Cards may be obtained at the Registrar’s Office. Hours of operation are:
- Monday - Thursday 8:00 a.m. - 6:00 p.m.
- Friday 9:00 a.m. - 5:00 p.m.
Anyone obtaining a new or replacement card must present photo identification such as a valid driver's license, passport, or military ID.
5. REPLACEMENT CARD
All students, employees and guests of the university are issued their first Lions ID Card free of charge. If this card, or subsequent cards, is lost, stolen or mutilated, a $20 fee will be charged for any replacement card.
Replacement cards may be obtained at the Registrar’s Office. Any request for a replacement card must be done in person, and the requestor must present photo identification. The replacement card will only be issued to the person identified on the card.
No person shall possess more than one Lions ID Card. If a new Lions ID Card is issued for any reason, the old card will be deactivated.
Temporary cards are not issued. Anyone without a card, but needing one for any campus service, will be required to obtain a replacement card from Records and pay the replacement card fee.
6. OBLIGATION TO REPORT LOST OR STOLEN CARD
The cardholder is obligated to report a lost or stolen Lions ID Card as soon as possible. This report may be made online through the UAFS website, or to the UAFS Registrar’s Office at the following location:

The student or employee is responsible for paying any applicable replacement fee prior to or concurrent with the reissuance of his or her Lions ID Card. The current replacement fee is $20.
All unclaimed cards turned in to the Registrar’s Office will be destroyed at the end of the current semester.
7. USE OF LIONS ID CARD AS IDENTIFICATION BADGE
All Lions ID Cards have a magnetic strip and bar code used for scanning purposes. For this reason, Lions ID Cards cannot be attached to a lanyard or clip by means of any physical modification to the card (i.e. hole punched in card). Student and employees who are required to wear their Lions ID Cards as an identification badge during duties performed on- or off-campus, including student internships or clinicals, should use a plastic badge cover or holder.
8. CARE OF LIONS ID CARD
All cardholders should follow the steps below to keep the card in good condition.
Never store the card in direct sunlight or expose it to excessive heat.
To avoid having the magnetic strip scratched by coins, keys or other objects, do not carry the card loose in a pocket or purse. Carrying the card in a plastic sleeve or cardholder is recommended.
Do NOT bend, fold or punch holes in card.
Keep card away from electromagnetic fields such as microwaves, televisions, cell phones and stereos.
Stickers, pins, tape or other items may not be affixed to the card.
9. RIGHT TO ALTER TERMS AND CONDITIONS
The University of Arkansas - Fort Smith reserves the right to alter any terms or conditions contained herein. A current version of this agreement is available upon request from the UAFS Registrar’s Office, or online at the UAFS website.
